I, the parent or legal guardian of the minor child named herein, hereby grant permission to Boston Film Academy (“BFA”) and Embassy Performing Arts Center ("Embassy") to photograph, record, or otherwise capture my child’s likeness, voice, and creative work during participation in Film Summer Camp workshops and activities.

I understand and agree to the following:

1. These photographs, videos, or recordings may be used by BFA/Embassy for promotional, educational, or archival purposes, including but not limited to:

• Marketing materials (flyers, brochures, digital ads)

• BFA/Embassy website and social media channels

• Press releases or local media coverage

2. My child’s first name, last name, and likeness may appear in such materials, but no other personal identifying information will be disclosed without additional consent.

3. This authorization is granted voluntarily and without compensation.

4. I may revoke this release in writing at any time; however, this will not affect any materials created prior to the date of revocation.
